This is a reminder that supervisors and non-supervisors who
participate in Jefferson Lab's STOP (STOPLight) program, or who have
done STOP observations, or who have been the subject of a STOP
observation are being asked to complete a survey about the program.<br>

The ESH&Q survey is hosted offsite by Survey Monkey and can be
found here:<br>

The survey is anonymous, however, at the end of the survey you can
choose to provide your name and contact information. <br>

The purpose of the survey to assist the lab in improving its safety
programs.<br>

The online survey consists of 17 questions and should take just a
few minutes to complete.<br>

You will have until Tuesday (Dec. 13) to complete the survey.<br>
