[APEX] updated APEX proposal for PAC 37

Dear APEX collaboration,

As you know, the APEX proposal has been conditionally approved at PAC 35.
 We are re-submitting the proposal to PAC 37.  The latest version of the
proposal can be found here:

Please let us know if you have any comments.  In particular, please check
the author list and let us know if:

1) you have not been listed as an author but would like to join the
collaboration
2) you should have been listed as an author but were accidentally left off
3) you are listed as an author, but would like us to remove your name

For authors, please check your affiliation and send us any corrections.

We look forward to the full approval of APEX during the PAC 37 meeting.
