[Casa] Anthem at-home COVID-19 test kits

Hi folks,

A while back I received an email from Anthem about no-cost home test
kits for COVID-19. This included a link to the web page:

This web page should be accessible to everyone -- I tested in a web
browser in privlate mode. You will have to log into Anthem upon clicking
"Order your test kit" on the web page above to order test kits, assuming
you are in a supported Anthem plan through the lab. When I logged in, I
could get two home tests per person in our household who is on our lab
insurance.
