Hi folks,
I'm passing along this thread about an electrical safety incident at
SLAC where a worker was injured very seriously and remains hospitalized.
There was some discussion of this incident at the 08:00 ops meeting
this morning. SLAC is currently shut down aside from administrative work
during the investigation. It would not be surprising if all national
labs see some reinforcement of safety training and awareness in light of
this serious event.
Though we in CASA mostly do not work much on highly energized systems
(aside from the magnetron and other devices), we all work in an environment
with many electrical, industrial, ODH, and occasionally chemical hazards.
Please exercise situational awareness, take your time when working in
potentially hazardous environments, and document and follow procedures
when appropriate.
As a personal aside, I sometimes do electrical work at home, and
use an inexpensive non-contact voltage detector to confirm that circuit
is de-energized before doing any work on exposed circuit elements.
This event reinforced to me the importance of this quick but important
step in even home electrical work. Electrical, fall, and other safety
awareness does not begin or end at site boundaries. Please be vigilant
and work safely.

Please see the report about serious electrical incident at SLAC.
Please notice the missing PPE in the report.
It happened on December 27, while Dec 26 was a holiday.
Coming back from holidays, some extra time may be needed to get back to 100% focus on work.
It also has similarity with electrical arc event at SLAC in 2004 and JLAB event in 2019.
Please discuss this event in safety minutes.
I also heard from Stuart at today’s Director Council that the injured person is still in hospital, but recovering.

Please see this attached ORPS report. This was a very significant arc flash event at SLAC between Christmas and New Year that we should pay attention to. Seems to be a classic Human Performance event. No doubt there will be much to learn when we see the full event investigation report.
