[clas_term] Important - Health Plan Survey

Dear Everyone,

As some of you may know, there is an ongoing discussion about the health
plan which is currently provided by JLab to its users. The time is rapidly
approaching for a renewal of the plan (or a decision to opt for another one)
and a questionnaire has been compiled to gauge opinions of its quality.
Please take a minute to complete the questionnaire (it is only one page),
*especially* if you are aware of the health plan and have ever made use of
it (or specifically decided not to):
