[Cuga] Personal Dosimeter Change Out is Dec. 19; Sign 2012 RWP by Dec. 31

Attention: All Jefferson Lab Radiologically trained individuals:

*Personal Dosimeter Change Out is Monday, Dec. 19 *

JLab's semi-annual personal dosimeter change out will take place during 
the afternoon of Monday, Dec. 19.  Please be sure to put your dosimeter 
in its place in the personal dosimeter racks no later than 1 p.m. 
Monday. This includes the dosimeters of all staff, students, users and 
subcontractors.

If you need to use your dosimeter during the change out period on Dec. 
19, you may exchange it that morning in the Radiation Control Dosimetry 
Office in Bldg. 52B, Room 4, 8 a.m. -1 p.m.  If you are leaving for the 
holiday break before then, be sure to put your dosimeter in its rack 
before departing.

*Read, Sign 2012 General Access Radiological Work Permit By Dec. 31.*

The 2012 General Access Radiological Work Permit is now available for 
signature. 

If you are a radiation worker, to retain your Accelerator Site access, 
you must sign the new RWP no later than Sunday, Dec. 31, 2011. To access 
the RWP, visit the web-based training page and open document SAF801kd 
General Access RWP (under the subhead "ES&H").

The JLab web-based training page is located at:

After reading the document, be sure to click on the line at the very 
bottom of the document that reads: Click here when you have read and 
understand this document. Fill in the required information fields and 
submit in order to document completion of training.

You may access/sign this document from offsite, but be aware that it 
won't be accessible on Friday, Dec. 23.

If you haven't completed the training, you won't be able to get through 
the Accelerator Site gate on Jan. 2.
