Visitor medical insurance renewal for 2012/2013 begins on October 1, 2012.
New policy information and insurance cards will be available for pick-up 
at the
SSC (formerly the VARC) front desk beginning Friday morning, September 
28, 2012.


After September 30, 2011-2012 insurance cards will no longer be accepted by
doctors and medical facilities. Please be sure to review all coverage 
changes and
 be aware of your insurance responsibilities.

If you have an Insurance Plan through the Lab, please fill out the new 
attached
HIPAA form as well as the correct Enrollment Form (either Researcher, or 
Student/PostDoc)
and bring them by the SSC.
