[Cuga] Visitor Insurance Renewal Information - A message from Tara Tyndall

Visitor medical insurance renewal for 2016/2017 begins on October 1 , 2016. New policy information and insurance cards will be available for pick-up at the SSC front desk (building 28) beginning Friday morning, September 30 , 2016. 

Old insurance cards will no longer be accepted at doctor's offices or medical facilities after September 30 , 2016. Please be sure to review all coverage changes and be aware of your insurance responsibilities. 

All visitor insurance plans include a MEDCO prescription discount and AXA travel assistance services. 

If you have a visitor insurance plan through the Lab, please fill out the attached HIPPA form as well as the correct enrollment form and bring with you to the SSC.
