    <pre wrap="">Visitor medical insurance renewal for 2017/2018 begins on October 1, 2017.  New policy information and insurance cards will be available for pick-up at the SSC front desk (building 28) beginning Friday morning, September 29, 2017. 


Old insurance cards will no longer be accepted at doctor's offices or medical facilities after September 30, 2017.  Please be sure to review your coverage and be aware of your insurance responsibilities. 


All visitor insurance plans include a MEDCO prescription discount and AXA travel assistance services. 


If you have a visitor insurance plan through the Lab, please fill out the attached HIPPA form as well as the correct enrollment form and bring them with you to the SSC when you pick up your new card.
