was approved in January, however the lab is still being assembled. No work has ever been done under the LOSP.</div>

<p class="x_MsoNormal"><span style="font-size:12.0pt">I have attached a revised spread sheet containing the OSPs that we need to write a return to work form for. I have also attached the form.

<p class="x_MsoNormal"><span style="font-size:12.0pt">No hazard code 3 or 4 work may be done under these OSPs until the OSP has a filled out form that has been reviewed by the owner of the OSP, their supervisor, the signers of the OSP, the DSO and Thia or her
designee. If some of these OSPs are no longer relevant please let me know and I will remove them.</span></p>

<p class="x_MsoNormal">In order to comply with the restart instructions from Johnathan and Stuart, all Physics Division group leaders must speak to their applicable people to ensure they understand how to calculate a risk code using the table
<a href="https://www.jlab.org/ehs/ehsmanual/3210T3.htm">here</a> in the ESH manual, fill out a task hazard analysis found
<a href="https://www.jlab.org/ehs/ehsmanual/3210T1.htm">here</a> in the ESH manual, mitigate the risk code to below 2 and incorporate the risk codes into your tasks.</p>

<p class="x_MsoNormal">All technical personnel in the Physics division should be familiar with the Task list system (Alist, Blist etc.) including where to find the appropriate task list (found
<a href="https://tasklists.jlab.org/">here</a>) and how to fill it out.</p>
<p class="x_MsoNormal"> </p>
<p class="x_MsoNormal">As per the Physics work governance document that we all must read, understand and sign, any task that is found to have a risk code above a 2 is required to have an OSP. Please ensure that all your machine tools, welding areas, power supplies
experimental equipment that has a risk code of 3 or 4, regardless of whether it is considered “Standard Equipment” or not and any other equipment that requires an OSP, has a current one. The OSP must be posted in the work area and signed by the people working
on the equipment. </p>
<p class="x_MsoNormal"> </p>
<p class="x_MsoNormal">The Subject of what is required for class 2 or 3 electrical equipment is still being defined and will be relayed when it is known.</p>
<p class="x_MsoNormal"> </p>
<p class="x_MsoNormal">We will be using the attached form to document for our recertification process of all premeditated risk code 3 or 4 OSPs and class 2 and 3 electrical equipment.</p>
<p class="x_MsoNormal"> </p>
<p class="x_MsoNormal">Your friendly neighborhood DSO will be available for questions and walkthroughs and be visiting groups throughout the day to answer questions and check on progress.</p>
<p class="x_MsoNormal"> </p>
<p class="x_MsoNormal">As per Johnathan’s email we are on a stand down for high risk work until we certify we have the proper controls in place. The above guidance will be required from all groups prior to recertification of the High hazard task.</p>
