Hi Brian,

Thanks for the information, I'll share these findings with Guido (support from CAEN).
I would think that this issues was with the mainframe/CPU of .136 crate but since you moved 19671 card (card with reported issues) from .136 into slot 2 of .135 and only appeared the PVs for the .135 mainframe then I could think that the cards are the problem.
I did not find any firmware file to download from CAEN website to upgrade the A1515BTG cards,  this might be something  to request to CAEN, if any newer is available.

Here's a quick run down of what I noticed moving the HV cards around between the crates.

There are 2 crates: caen-cnu-sbs (129.57.37.135) and caen-gem-c2 (129.57.37.136) I'll just refer to them by the last octet of the IP from now on.

.135 (HiVoCS = 1.4.0 / HV Control = 1.3.2) has 5 modules installed four A1515BTGHP in slots 0, 1, 3, 4 and a single A1515BTG in slot 5 (s/n 27135)
.136 (HiVoCS = 1.6.6 / HV Control = 2.1.1) has 2 modules installed both A1515BTG: one in slot 2 (s/n 27077) and one in slot 4 (s/n 19671)

With these modules installed .135 shows PVs for the mainframe and all the cards while .136 only shows PVs for the mainframe, but neither card.

When moving card 27135 from .135 to .136 into slot 5 and removing the cards from slot 2 and 4 (so only one card is installed) and power cycling the crate .136 now shows PVs for the mainframe and the card.

I then removed this card and only installed 19671 in slot 4, this also showed PVs for the mainframe + card.

Next having only 27077 installed slot 2 showed PVs for the mainframe + card.

Since it appears at this point the issue is having more than one A1515BTG installed in a crate I moved 19671 from .136 into slot 2 of .135, once I did this now the only PVs that show up are the mainframe, i.e. no card PVs appear.

I will note that all the A1515BTGHP cards have firmware 1.01 while all the A1515BTG cards have 1.00

Hopefully that will help CAEN narrow down their troubleshooting some. Let me know if there's any questions.
