<p>The DSG is reviewing your changes, so you may receive
responses/suggestions from anyone in the group.<br>

<p>I looked over the documents and have seen the changes. In the
OSP, you have the highest pre-mitigation risk code as a 2, which
doesn't correspond to the second step in the THA (3). I would
suggest two things for this. <br>
</p>
<p>1. The THA for this should perhaps be titled "Operating the LTCC"
and only list steps needed for operation, in sequence, and analyze
each for potential hazards. You actually have a sequence in the
OSP. This is a good place to start. Flowing gas is part of the
operational steps, here is where a reference to the gas manuals
will come into play. You should go over the gas manual information
to ensure the correct risk code will be used for the THA, again I
recommend leaning on Ed and Bert for guidance on risk code
assessment.</p>
<p>2. THA step two is listed generally as "servicing detector
components". If there are known maintenance/services
(ie...checking for leaks) which require the use of lifts (or any
other safety items...PPE, LOTO ect.), you should write a separate
THA for those individual tasks, and add them to the OSP package. I
would <u>not</u> have this in the THA to operate the detector,
but please consult with Ed and Bert before removing it.<br>
</p>
<p>Additionally, I see you have listed ODH training in the OSP.
Which sounds like the right approach. The LTCC is a large gas
volume in total (detector, tank, lines...ect). I would suspect the
need for an ODH review (which needs to consider all potential gas
in the Hall). You should consult with Ed and Doug Tilles, they
will have the information to perform the calculation (or possibly
may have already completed the calculation...maybe with the LTCC
volume included). Information on completing the ODH review is

<p>It is also common to have an area sketch/drawing as part of the
OSP/ODH review package.</p>
<p>The gas manuals are in editing but can certainly be linked and
updated as they are completed.</p>

THAs written as an operational procedure, each block
should be a step of "significant"operation which gets

but you didn't propose a mitigation for step 2, which
has a risk code of >2. This looks to be a broad
category, which may require a THA for servicing
detector components on its own.</p>
<p class="">For step 3, George has provided an operation
manual for the gas system and the DSG has a section of
a manual for specific operation of the gas controls
system. You should refer to these documents for gas
operations.</p>

timing/amounts of flow (ie...metered purges). There
are also complex steps to recover C4F10 using a

to the OSP which will ensure named trained Hall B
staff will be used to do the operations listed in the
manuals.</p>
