Thank you for making a detailed record of this. 

It looks like the 'physical damage' (where the copper braids enter the mounting block) has been there since day one - possibly when the leads were being installed (Mid 2016 I believe). 
I inspected the leads on delivery from the vendor and no physical damage was noted at that point. Solder over-spill on the braids was noted - as a result of the vendors manufacturing process. 

This may also explain why there are copper whiskers within the cabinet - as a result of the lead installation last year. 

However, if copper whiskers are still coming off of the copper braids - this is obviously not acceptable. I shall review how we can prevent this from happening. The braids must stay exposed to the air for cooling purposes so we cannot simply seal them. 

We will need to look at the solenoid power supply cabinet too.
