There are two important safety topics I need to review with you and
have you act upon.
<br>
<br>
<b>*1) LOTO: Admin and Personal (Danger) Lockout Tag Changes*
</b><br>
<br>
The newest version of the Danger and the Administrative LOTO tags are
now
<br>
available through the stockroom. The supply of 1000 that was in free
stock has been
<br>
exhausted so another 1000 have been ordered and should be on site
somewhere around November 9-10.
<br>
The goal is for each group to sweep their areas of
<br>
responsibility and change out _<b class="moz-txt-star"><span
class="moz-txt-tag">*</span>all<span class="moz-txt-tag">*</span></b>_
tags of active lockout/tagouts with the
<br>
newest style tags. The newest style tags do have slight changes, which
were a
<br>
result of the 2009 LOTO audit, and it is important that every tag get
replaced;
<br>
even those in your LOTO supply areas.
<br>
<br>
When the tags in free stock run out, then individuals will have to
place their
<br>
own order through the stockroom. The stockroom can order tags that have
either
<br>
metal grommets or plastic grommets for use in sensitive areas.
<br>
The part numbers for the tags with metal grommets will be:
<br>
610050020 (Danger Tag)
<br>
610050030 (Admin. Tag)
<br>

As Engineering DSO I want to be sure that we take the time to
thoroughly sweep our areas of the old tags. I know that many of you
have already started this process; thank you. However, I want to be
sure that everyone takes time out from their normal duties to perform
this sweep. So, I want each work group to designate the afternoon of
Wednesday, November 4 as a time to do this sweep to the extent
possible. (doing it ahead of time is of course ok). When your group
finishes its sweep, designate one person to report the results, what
problems you had such as you couldn't get into the tunnel to exchange
tags, you ran out of tags and couldn't get through Bldg XX, etc. to
John Kelly at X7531 or email <a class="moz-txt-link-abbreviated"
href="mailto:jkelly@jlab.org">jkelly@jlab.org</a>. I realize that we
may need to do this sweep in stages since we are out of tags but I want
to at least keep track of what has been done. In your walks I'm sure
you will encounter other peoples' tags that need to be changed out. Of
course that is their responsibility but letting them know that you have
found an old tag would be the neighborly thing to do if you believe
they may have missed it.

<b>*2) Experimental Hall Access for work*</b>;
<br>
<br>
Recently there have been questions about accessing the Experimental
Halls for repairs and maintenance after an experiment has been staged
and is in its scheduled running time. Currently if you are going to
access one of the Halls under controlled access you must have read and
signed the COO, RSAD and ESAD documentation specific to that experiment
(these are available in the counting house or here: Hall A: <a
class="moz-txt-link-freetext" href="http://hallaweb.jlab.org/">http://hallaweb.jlab.org/</a>,
go to "Current Experiment" and click on the experiment number, under
that go to "Run Information" and click, you will find the documents on
that page, Hall B: <a class="moz-txt-link-freetext"
href="http://www.jlab.org/Hall-B/">http://www.jlab.org/Hall-B/</a>, go
to "Current Run Information" and look for these documents, Hall C: <a
class="moz-txt-link-freetext" href="http://www.jlab.org/Hall-C/">http://www.jlab.org/Hall-C/</a>,
go to "Current Run Information" section and you'll find the documents)
, or you must be escorted by a Hall representative who has read this
documentation and signed it. If the Hall is in restricted access you
may enter the hall unescorted if you have the usually required
training. (i.e. General RWP, Hall Specific training, etc.) Of course
all work must be coordinated through the appropriate Hall Work
Coordinator and have an ATLis or Hall List entry to cover the job.

Also, just today a "Conduct of Installation" was introduced for the
installation of Qweak in Hall C. It is located here: <a

under "Upcoming Run and Installation Information". If you are
accessing Hall C during this installation you must have read and signed
this document. It is located in the Counting House or MCC. We should
anticipate similar documents in the future for installations in all the
Halls with a requirement to sign it yearly, like the General Access
RWP. So stay tuned for information on that procedure.

<b>Finally</b>, congratulations are in order for everyone. In case you
didn't know, the lab ended FY'09 with a DART (Days Away, Restricted
or Transferred) rate of 0. Only one other lab has ever done this,
(Ames Research for 2 years straight) and it is a credit to your care in
planning and executing your work that makes this possible. Great Job!
and keep it up. You help make both the Engineering Division and the
Lab look good but more importantly a safe place to work.<br>
