I'm confused about the 4 lines on the form that I'm completing.
I don't seem to be able to find a copy of the revised contract.

If the contract 4 lines is not the complete construction and delivery of 4 prototype coils I may have the form filled in incorrectly.

Submittals were requested by COB today.  It is imperative that we receive a response by noon Monday, 12/05/2022.

Please complete the Subcontract Percent Complete Form (PO Accrual Form) for work completed through November 30, 2022. It is important that we receive estimated percent complete forms associated with work performed through this date for which we have a contractual obligation where an invoice has not been received and approved.
