Statement of Lessons Learned 
A questioning attitude helped spot a non-compliant fall arrest apparatus. Discussion of Activities 
A worker questioned the amount of resistance needed to extend the webbing of a Miller MiniLite self-retracting lanyard (SRL), manufactured in April 1999.  Upon examination it was determined that the SRL was not functioning properly, and it was removed from service.  During the review it was also recognized that the carabineer used was of a twist locking variety, which is not permitted by today�s standards. Analysis 
By all outward appearances the SRL and associated parts were in good shape and properly maintained (Figure 1). 

When originally purchased in 1999, twist-locking carabineers (Figures 2 and 3) were acceptable and provided (see Attachment 1) as standard equipment.  Now the use of twist-locking carabineers is not permitted. Recommended Actions 
1.	Provide notification to all individuals who are trained in Fall Arrest 1.  Equipment that twist-locking carabineers are no longer permitted.  If one is identified in service do not use it, notify the Material Handling Safety Representative (MHSR) (x7406) for a replacement
2.	MHSR is to inspect all SRL equipment and replace all twist locking carabineers with self-locking ones. JLab Preventive Measures
