I need the attached form completed for the people that want or need to 
work during the shutdown. I have sent this to the Hall and Group leaders 
and also to supervisors. Please enter the information for your employees 
as you see, has already been done for a few employees. I need this 
information by Wednesday at noon, December 17. If you or your employee's 
name is not entered on the list, you will not be allowed to enter the 
lab during the shutdown. If you need help, please contact your support 
person, but be sure she receives all the relevant information.
