[Halld-offline] CCDB updates

Hi Offliners,

As discussed in last week's Calibration meeting, I'm planning on adding the
following tables to CCDB & sim-recon to allow for the handling of
noisy/dead/etc. channels.  Please let me know if you have any comments or
changes before Wednesday.
