[Halld-offline] Changes to mcsmear in sim-recon

Hi all,

I've just submitted a pull request to merge some of the changes that I've
been working on for mcsmear, which I've mentioned at several recent
meetings.  The major change is a reorganization of the code, since some of
these features added enough lines of code that the current structure was
starting to become unwieldly.

I've run several tests, and the only practical difference that these
changes should make right now is that "realistic" channel-dependent time
resolution parameters are now being used to smear the TOF and Start Counter
hits.  Hopefully this new structure will make changes easier as we update
the smearing parameterizations for all detectors.

One caveat:  I did remove some code that was, as far as I could tell,
deprecated.  If I removed a feature you need, please let me know and I can
easily add it back in.  Please let me know if you have any other questions
about these changes.
