[Halld-pid-upgrade] DIRC bar dimensions and notes

Hi all,

I added a final document that I received from Jerry that contains dimensions of individual bars as provided by Boeing during the QC process.

As Yunjie noticed, the sheets from Jerry only include information about dimension in the y direction (as installed in GlueX) as this was critical for getting proper shims to avoid mechanical stress when the assembled bar box is rotated about it long axis.
