                 <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
                 <pre>Recently, while in the process of installing a bulkhead for a temporary containment structure, a worker drilled into an energized 440 VAC shipboard cable located behind a bulkhead’s sheet metal structure. </pre>
                
                 <h2 style="white-space:nowrap">Discussion of Activities</h2>
                 <pre>The employee did not recognize that the cable had shock and arc flash hazard potential. The only concern was to avoid damaging the cable.
The employee was using a small drill to drill holes required for sheet metal rivet installation. Prior to drilling the first hole the employee moved the cable out of the drilling path. The first hole did not line up correctly so the employee drilled a second time to widen the hole. This time, however, he neglected to move the cable from the drill path. The drill bit entered the energized 440 VAC cable generating an arc-flash, which melted the drill bit, and causing a short circuit. Fortunately, the employee was not shocked or injured from this incident.

                 <h2 style="white-space:nowrap">Analysis</h2>
                 <pre>The employee was not an electrician and only recognized that the drilling operation could cause damage to the cable. If the employee had known the cable was a potential electrical hazard the situation would have been evaluated differently. The cable would have been properly secured and controlled for the duration of the drilling operation.
The electrical shock and arc-flash hazard could have resulted in the loss of this employee’s life.
