                 <pre>1. Report all unwanted events (example; damage to equipment) immediately to your supervisor and to the applicable division/department/group (example; FM&L, Security & Services Group).
2. Do not attempt repair on material handling equipment, however, immediately contact your supervisor and the Facilities Management & Logistics Division, Security & Services Group.
3. Use Jefferson Lab EH&S Lock Out Tag Out and Try procedure when there is a need to take material handling equipment out of service.

                 <h2 style="white-space:nowrap">Discussion of Activities</h2>
                 <pre>On Monday, November 25, 2013, a material handling service technician from the Facilities Management & Logistics Division/Security & Services Group discovered an unauthorized disassembly of a control box from an aerial platform / personnel lift located in the Experimental Equipment Laboratory (EEL), Room 125.
Prior to discovery the lift was being used with known inconsistencies to the operational control and a structural support defect, and there were several opportunities for the lift to be properly taken out of service and properly repaired, but neither happened immediately.

                 <h2 style="white-space:nowrap">Analysis</h2>
                 <pre>Root Cause: Unauthorized maintenance of material handling equipment.
Contributing Causes:
1. Failure to immediately report defective/damaged material handling equipment to the supervisor and the FM&L Division, Security & Services Group.
2. Failure to properly tag out defective /damaged material handling equipment

                 <pre>1. Complete material handling equipment repairs on control assembly unit and mounting bracket.
2. Ensure Lessons Learned is shared with material handling personnel.</pre>
