Moving heavy or unusual items require a thorough hazard evaluation to prevent injury.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On September 20, 2013, a Fluor-B&W Portsmouth employee injured his left bicep tendon while moving a fire proof file cabinet. The employee was positioning the file cabinet on a truck lift gate with the assistance of another employee when the injury occurred. The employee was treated onsite and transported to a local hospital.
On December 11, 2013, it was determined that surgery was required to repair damage to the tendon making the incident reportable in the Occurrence Reporting and Processing System.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The worker was part of a composite crew that was being used to move office and maintenance equipment. The crew was made up of multiple crafts with some members working out of their classification. File cabinets were emptied of their contents to make them easier to maneuver. Management evaluated the hazards associated with the task and determined it could be performed under the General Site Job Hazard Analysis (JHA). The evaluation and the General Site JHA were used to perform the work, but neither consider the additional weight associated with moving the fire proof filing cabinets.
The following causes lead to the injury:
-The crew was comprised of individuals who were not accustomed to utilizing proper lifting techniques.
Two people was not enough for lifting heavier items
There was no walk down prior to work to identify the heavy or awkward equipment.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Work was stopped to determine how best to proceed without injury.
Walk down with employees was held to determine best approach for work going forward.
Hydraulic Lift-and-Move Fork Dollies were purchased for future moves on the project.
