<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Supervisors should regularly talk about Suspect Counterfeit items, encouraging discussions and questioning attitudes within their workforce. </pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>During a ESH safety warden inspection on March 10, 2014 of the TEDF Klystron/RF 1519 (2) Stainless Steel 3/8” Shackles were identified as potential suspect counterfeit items because of the China markings. The shackles were not being used in activities at the time of discover and removed by the ESH safety warden walk through team.
The employees contacted the SC/I Coordinator with a photo (see attached) of the suspect 3/8” stainless steel shackles. On the morning of March 11, 2014 the shackles were reviewed and confirmed as suspect counterfeit items because of the China markings and the lack of manufacturer identification and working load limit required by ASME. ASME B30.26 addresses Rigging Hardware and requires a name or trademark of the manufacturer, rated load, and a size. HSS training and the JLab training also require the same markings. Without the proper identification of the manufacturer and working load limit, the SCI Coordinator cannot accept the rigging shackle as a usable device for lab activities.
During the review of the initial (2) 3/8” stainless steel shackles, the area was checked again and (2) additional shackles were turned over by the supervisor to Tina and Todd which were turned over to the SC/I Coordinator. There are now 4 stainless steel 3/8 shackles in possession of the SC/I Coordinator which will be disposed of to ensure no use on site.
TJSO notified the Office of Inspector General of the findings on 03/12/20014. On 03/18/2014 the Office of Inspector General sent a memo (memo is on file) stating that we can release (properly dispose) these shackles at the SCI Coordinator’s discretion.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>Root Cause: Source of the Suspect Counterfeit item could not be determined; therefore we were unable to determine a root cause for this event.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>1. An email was sent to all Safety Wardens requesting that they look in their work areas to see if they may have the above pictured shackle in their work area. No additional shackles were reported.
2. Order replacement rigging devices that meet the SME code requirements for the work groups effected by the removal of the 4 shackles
3. Shackles were entered into the Suspect Counterfeit database (entry #301) and there were appropriated secured.
