Situation awareness while working around office equipment is essential to ensure a safe work environment is maintained. All potential hazards must be considered prior to performing any task.
</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On April 7, 2014, an employee accidently energized a shredder machine while attempting to plug in a desk fan. The employees’ badge was inside the shredder’s feeder when it started up. The lanyard, used by the employee, provided by Hanford, was designed to break away. In this case the lanyard separated preventing injury to the employee. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The employee had recently received the shredder to replace a smaller unit. While reaching past the unit to plug in another electrical device the employee’s arm brushed the switch of shredder. The badge of the employee was dangling in the shredder’s maw when it began to run. The employee had a startle reflex pulling back from the machine when the breakaway feature of the lanyard separated, releasing the employee from the badge and preventing injury.
The new machine, with controls located in a significantly different position, and associated tasks created an unsafe condition.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
To prevent recurrence:
• Always wear lanyards that are capable of breaking away should they get caught on machinery, ladders, doors, handles, etc.
• Be aware of all office machines and their point of operation.
