Statement of Lessons Learned

1. It is recommended that employees are screened by Occupational Medicine to ensure that they are “fit for duty” prior to being placed on the team.
2. Task rotation when doing physically demanding tasks jobs should be a high priority.

Discussion of Activities

An employee was asked to come to work to shovel snow as part of a mission essential work team. The employee felt well until next morning when he awoke with low back pain/soreness. Over the next few days the pain steadily decreased until there was a second snow day. Again employee shoveled snow at work even though his back was not yet well. This second snow day made his back much worse and so on March 5, 2014 he notified his supervisor and decided to seek medical attention at an urgent care center.

Analysis

Root Cause:
Work Planning was LTA- Team members were assigned to repetitive physically demanding tasks for the entire duration of their shift.
Contributing Causes:
Snow removal activities led to muscle fatigue and soreness. Soreness was believed to be “normal” and therefore went unreported.
Failure to report initial soreness to supervisor in a timely manner.- Past experiences with event reporting (approximately 13 year ago) may have impaired the employee’s choice to delay reporting his soreness.
During recent refresher training, employee was not present when “Reporting” was discussed and therefore may not have had a clear understanding as to when he should report to Occupational Medicine.

Recommended Actions

1. Hold a staff meeting to re-iterate the importance of reporting soreness to your supervisor in a timely manner.
2. When discussing the work plan for snow removal, ensure that there is task rotation every couple of hours.
3. Request Occupational Medicine review the “Snow Removal Team” members to ensure all members on the list are “fit for duty.”
