Lifting packing material from a drum can cause injury.

                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>An operator was injured while lifting packing material from a drum. The packing material was divided into three sections and fit snugly.  The operator was in the process of removing the middle section, which was the biggest and heaviest, when the injury occurred.

The middle section weighed ~40 lbs and was routinely removed manually.   In order to get a good grip on the material operators would insert their hands and exert outer force between the material and the drum while pulling straight up, putting strain on the back and shoulder.

As the material was being lifted from the drum, it slipped from the operator’s hand. When he repositioned his hand to catch the material he felt a "pop" followed by pain in the left shoulder.

                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>A “Barrier Analysis” was performed to assess existing physical and administrative barriers used to prevent injury to the operator. This analysis identified multiple physical and administrative barriers that were in place and functioned as-expected.

Operators were trained on work being performed, proper tools were used. All identified barriers functioned as expected.

There was one barrier missing.  This included an alternate tool or method for lifting the packing material out of the drum.

                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>Below are the consequences of the above event.  They are provided for information and discussion purposes only. 

An Extent of Condition review was conducted looking at other work activities involving lifting and pulling performed by operators.

The supervisor was tasked with notifying operators that placing the drum on its side and then sliding the material out of the drum, instead of pulling it out of the top, was an acceptable method and would reduce the risk of potential injury.
