An annual Lockout Tagout Review is required by the Occupational Safety and Health Administration (OSHA) and the Washington State Department of Labor & Industries to ensure staff understands and follows LOTO procedures and requirements. The objective of the review is to identify deviations or inadequacies in the program.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>During an annual LOTO review, a team conducted field inspections to evaluate PNNL’s Hazardous Energy Control Program. They found that requirements were generally being followed, however, they did find two tags that did not meet expectations.
One of the tags was incomplete and missing required supervisor and organization information. The second tag had become weathered and was difficult to read. All of the issues identified were related to construction activities. Construction managers were notified and immediate corrected the issues.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>Safety tags for LOTO are prepared and installed to communicate hazards and important safety information. Accurate and complete information is necessary in the event the authorized worker needs to be contacted to change the LOTO condition or if removal is required. OSHA regulations for tagout devices require them to be:
* Durable: Tags are to be constructed and printed so that exposure to wet or corrosive conditions does not cause them to deteriorate or become illegible.
* Complete: Tags are to be filled out completely to communicate all required information clearly.
* Legible: Tags are to be filled out in a way that makes them easy to read and understand.

Authorized workers who install Lockout and Tagout (LOTO) tags are required to follow instructions and make sure they are filled out clearly.
