Analyzed the hazards associated with operating vintage equipment (e.g. overhead bridge cranes) and the use of historical practices (riding the bridge) against current requirements to ensure work is performed in a safe manner.

                 <h2 style="white-space:nowrap">Discussion of Activities</h2>
                 <pre>Original overhead bridge cranes at the DOE Portsmouth site were installed in the late 1950’s. Since that time, OSHA instituted 1910.179, "overhead and gantry cranes" Section (e)(6)(i)-(iii) specifically addressing the requirements for guarding moving parts on an overhead crane. The vintage overhead bridge cranes had guards over the couplings, but did not have guards over the cross shafts. Based on a clarification between the local OSHA office and FBP Health & Safety, it was determined that the cross shaft should be guarded as well.</pre>
                
                 <h2 style="white-space:nowrap">Analysis</h2>
                 <pre>The historical practice of riding on the crane bridge platform had not been evaluated relative to current OSHA safety standards and requirements. Recent pre-job planning, which involved the use of one of the vintage cranes as a working platform, called into question the historical practice.</pre>
                
                 <h2 style="white-space:nowrap">Recommended Actions</h2>
                 <pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
In the past Code Inspectors have stood on the ends of the bridge walkway as the cranes moved slowly down the length of the rails. Either of the following options is being considered to mitigate the hazard.
Engineered Controls: Install machine guards on the bridge cross shafts to protect personnel who are inspecting crane rails from the moving bridge platform. The guarding must be compliant to 1910.179 and 1910.219 and be of sufficient length to prevent an employee reaching the point of operation, rotating shaft, etc., even if incapacitated. Depending on the equipment configuration, the guards may need to be specially fabricated or might be bought off the shelf (e.g. Falk). Install Anchor Point (D-rings) for use as fall restraint to restrict potential exposure to any unguarded portions of the rotating shaft during inspection activities.
Eliminate the Hazard:, Engineering is investigating the use of a sports action camcorder to perform the annual OSHA bridge rail inspections. The camera is compact and rugged, with high resolution. It comes with a waterproof case that could be easily decontaminated. The cameras would be mounted on the crane’s end truck not in the photographer’s hand. The footage can be played on a large HDTV screen. The camcorder is inexpensive at a few hundred dollars.
