                    <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>

During an ergonomic evaluation in her office, a staff member turned off a floor lamp she had brought from home and a four- to six-inch flame that could have burned her hand shot from the side of the socket base. Acting swiftly, a Worker Safety & Health representative pulled the plug from the wall, extinguishing the fire and promptly called security to report a minor electrical fire. The breaker did not trip and the plug and wall outlet were undamaged, however, the light bulb and socket were badly charred.

                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>The lamp was marked with misleading labels, implying it was UL-listed. It is PNNL’s policy to make sure that electrical item/appliance are certified by Underwriters Laboratories (UL) or another Nationally Recognized Testing Laboratory (NRTL) as some UL or NRTL labels may be counterfeit. </pre>
                  
                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>A staff member in PNNL’s Fire Protection Program searched the Consumer Product Safety Commission website for the model or type of floor lamp, but found no results. The lamp had a misleading/counterfeit white sticker on the bottom of the base that read "UL Model LH-1020R," but did not have a UL label. Similar lamps have been found in other offices at PNNL.</pre>
                  
                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>Below are the consequences of the above event.  They are provided for information and discussion purposes only. 

When bringing personal electrical appliances from home to use in your office or laboratory, make sure they are safe to use before turning them on.

All staff members

* Inspect your appliance. Verify that electrical appliances or equipment brought to PNNL are in good condition. Before using them, check cords, plugs, bulbs and other components for wear or defects. Avoid running cords through openings in doors, walls, ceilings or under carpets. Avoid stringing power strips together.

* Verify the listing. Make sure all electrical appliances used at PNNL are certified by an NRTL such as Underwriters Laboratories (UL). Be familiar enough with authentic UL or other NRTL labels to recognize when a certification label is questionable, especially in low-cost, low-quality products.

* Report questionable or missing NRTL labels. If you bring a lamp or other electrical appliance to work, that does not have a UL label or similar NRTL label, contact the electrical safety Subject Matter Expert, for an inspection.
