The actions taken after an isolated scaffold pole fitting failure demonstrate and re-enforce the concepts of a questioning attitude and taking a time-out-for-safety to investigate abnormal events.
</pre>
                
                 <h2 style="white-space:nowrap">Discussion of Activities</h2>
                 <pre>On April 18, 2014, the male end fitting of a tube and knuckle scaffold system broke during installation. The failure was immediately reported to supervision and safety. The fitting failed at a location internal to the scaffold tube such that the normal inspection process could not have identified the problem. Based on the location of the failure and the unusual appearance of the break point, construction management performed additional follow-up activities to better characterize the event and mitigate any potential safety hazards.</pre>
                
                 <h2 style="white-space:nowrap">Analysis</h2>
                 <pre>Several scaffold poles with similar manufacturer labels were cross-sectioned along with a few from different manufacturers. The manufacturer was contacted and the presence of a representative was requested on site.</pre>
                
                 <h2 style="white-space:nowrap">Recommended Actions</h2>
                 <pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
The failed fitting and two randomly selected scaffold poles with the same manufacturer lot number were sent to an independent lab for destructive analysis.
At the conclusion of the independent lab analysis, the two poles were determined to be within acceptable tolerances. The direct cause of the failed fitting was determined to be corrosion. The initiating event that resulted in the corrosion could not be determined.
