<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>When completing a task for the first time, it is imperative that you take your time, analyze all the hazards, and perform the work in a safe and deliberate manner. </pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On October 14, 2014 a RadCon Department employee was checking an environmental dosimeter outdoors when they struck their head while attempting to pass underneath an elevated section of cryogenic pipeline (They had successfully went under the pipe to access to dosimeter 5 minutes prior to this event). The worker experienced some dizziness, nausea, and headache, and was transported to the hospital for evaluation.
The worker was diagnosed as having sustained a concussion. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Root Cause: Brief Mental Lapse. Employee successfully went under the pipe to access the area near the bottom of the stairs. Approximately 5 minutes later, she promptly turned around, forgetting the previously identified hazard (cryogenic pipe) and hit her head.
Contributing Causes:
(List as many as apply.) The pipes were not marked or padded with a caution tape and padding at the time of the event. Although not a regular walking path, the padding and caution tape may have raised enough awareness that the employee may not have forgotten the hazard.
This was the employee’s first time conducting this type of field work. She was unfamiliar with all of the areas and potential hazards within that area.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>1. Evaluate the possibility of moving the environmental dosimeter (Near Hall D Counting House) to a more user friendly location that satisfies RadCon specifications (location for easier change out and distance from the hazard).
2. Place weather proof foam and cautionary tape approximately 6 feet across the cryogenic pipe near the Hall D Counting House where this event occurred.
