Statement of Lessons Learned

For a copy of the original report, see “Attachments” on Lessons Learned page.
Tires lose air pressure when temperatures drop. As we move into colder weather, it is important to pay attention to vehicle maintenance and respond when there is problem. Look for indications of tire wear and low pressure when performing 360 walk-arounds and pre-use inspections.

Discussion of Activities

An employee at the Savannah River Site (SRS) said he noticed that the tire pressure was low on a government vehicle but did not think it would be a problem driving to the air compressor. Once he arrived at the air station, he reviewed the posted safety rules, donned safety equipment, and started filling the tire. When the tire was partially full, the sidewall split making a loud noise and startling the employee. The employee immediately stopped the work, shut off the compressor, notified his supervision, and a facility representative. There were no injuries to personnel but the tire was destroyed.

Analysis

Facility management responded and reviewed the scene. It was noted that the "low air pressure indicator" on the vehicle’s instrument panel was on. It was evident from wear marks on the sidewall of the tire that the vehicle had been driven with the tire severely deflated. The remaining tires were checked and all were found to be between 4 and 9 pounds low.

Recommended Actions

Below are the consequences of the above event. They are provided for information and discussion purposes only.
• Heed the warning signals on vehicle’s instrument panels and respond when there is problem.
• Look for indications of tire wear and low tire pressure when performing 360 walk-arounds and pre-use inspections.
• Do not operate a vehicle with low tire pressure. Notify Fleet Management immediately

JLab Preventive Measures

Jefferson Lab has a comprehensive Material Handling Equipment Program which includes routine inspection and maintenance requirements. Prior to use of any piece of material handling equipment or government vehicle a “Pre-use Inspection” is required, which includes a 360o walk-around. Operators are required to contact the Material Handling Safety Representative when defects or deficiencies are encountered.
