                    <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
                    <pre>By utilizing a GoPro® video camera to capture a point of view that would be impossible to see otherwise, project personnel were able determine how waste container cables were coming disengage from the container hook.  The solution was able to be repeatedly tested prior to retrofitting over a thousand containers with the fix.</pre>
                  
                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>A fix for the problem of roll-on/roll-off containers’ D-rings coming unhooked was to weld a "thumb" to the hooks (see attachment for photos).  This helped keep the cable connected when the primary locking mechanism was not engaged.  Most of the thumbs were effective in mitigating the problem, but there continued to be intermittent problems when the D-ring and container hooks were re-designed.  As a result, the original thumb was no longer sufficient to stop cable disengagement and the problem continued to occur.  In one instance, a waste container became disconnected and, when the truck resumed forward motion, the container rolled freely off the rear.  In another instance, an empty container rolled freely off a truck during unloading.  A causal analysis was unable to determine why the D-ring would disconnect from the hook on the container.</pre>
                  
                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>The challenge was to find a way to see what was causing the D-ring to come off the hook.  Project personnel mounted a GoPro® video camera on the front of a container and positioned the container so they could see how the cable and D-ring reacted during various configuations.   The video allowed personnel to see the mechanics of the D-ring detaching from the waste container. The video showed the D-ring on the end of the cable slide backwards, rotate upwards, and fall off the side of the hook when slack was in the line.

Armed with this information, a new design was created, which re-positioned the thumb, to catch the D-ring in the event of slack in the cable and hold it in place.  The original thumb location was on the top container hook.  The new location was toward the front of the hook.

The GoPro® was again used to evaluate the efficacy of the new thumb design prior to installation on approximately 1,100 containers. The video showed the new thumb repeatedly trapping the hook, preventing it from rotating.

                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>Below are the consequences of the above event.  They are provided for information and discussion purposes only. 

Approximately 1,100 containers received the thumb modification between May and September, 2014. Since the modifications were made, there have been no new issues with cable disengagement.
