                    <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>

When re-ordering a piece of equipment confirm the Manufacturer’s number along with the Model/Part Number of the original, since it is possible that the supplier made significant design change(s) since the original purchase.  These could cause form/fit/function issues.

                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>A replacement printer was recently ordered using the same model number as the one currently in use.  The printer was supplied (see new printer photo) was different from the original (see old printer photo) even though they were the same model numbers.</pre>
                  
                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>The new printer worked differently from the original.  As an example: in place of a "Print" button, the new printer had a "Release" button; and the new printer required additional steps to use it properly making it less "user friendly."

Both printers had a dipswitch to control how the printer worked.  The supplier said that the new printer used the same settings as the original one - this was tried and the printer did not print correctly. The piece of equipment that “talked” to the original printer was no longer configured properly.

                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>Below are the consequences of the above event.  They are provided for information and discussion purposes only. 

Do not order equipment by Model/Part Number alone; include the "manufacturer’s" part numbers as these are usually consistent in form/fit and function over the life of the product and a description of the intended use. 

When an exact replacement is necessary conduct market research to ensure that form/fit and function are identical to the item being replaced and the item meets all intended requirements.  Request documents such as product description, specifications, drawings, photos, operating manuals, etc., to assure it is an exact match.  In this case, a picture and the operating manual would have revealed the differences.  It is the responsibility of the end user to determine if the item offered by the supplier is an exact replacement.  If no "substitutions" are allowed state this on the requisition.

        <h2 style="white-space:nowrap">JLab Preventive Measures</h2>
        <pre>This is good advice for those who requisition items.  In this example it took over a week to resolve the issue and reconfigure the equipment properly.</pre>
