<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>The C-420 project includes the construction of a separation wall in a depressed area to collected storm water to be consolidated on one side and flowable fill added to the other. During work planning meetings several discussions were held concerning working in and around the contaminated water, including using only government-owned (not leased) equipment in the contaminated water, how the depth of the water would affect equipment, use of spotters to help assure the equipment operator did not allow water to enter the cab, possible methods to retrieve inoperable equipment should it become necessary, and possible methods to rescue stranded operators from inoperable equipment.
The work crew understood the hazards and precautions based on these discussions, but the information was never documented into the Task Work Instruction (TWI). During actual execution of the work, observers questioned whether the hazards had been considered.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Because the TWI did not contain documentation of the work process safety issues discussions, a work pause was instituted to incorporate the information.
All issues addressed during planning sessions needed to be documented and included in the final product before work was able to restart.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab has a robust Facility Design ad Modification Review process, outlined in ES&H Manual Chapter 3110. A Safety Review is required for new structures; architectural, structural, mechanical, plumbing or electrical system modifications; and changes in building use. All comments are addressed and recorded prior to commencement of work.</pre>
