<pre>Questioning attitude of new equipment revealed faulty anchor point for fall protection.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On March 17, 2015, maintenance mechanics questioned the location of the fall protection anchor point on a new JLG, Model #12SP manlift during performance of the inbound preventive maintenance. The maintenance mechanics and Safety and Health workers researched the operator’s manual and found that the anchor point attachment was not installed as identified in the manual. There is no way to disassemble the cage to place the anchorage point in the proper orientation as the entire cage is welded. A new platform and cage could be easily installed by removing four bolts to remove the faulty cage and platform. The new cage and platform could then be installed. A nonconformance report (NCR) was completed. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Extensive evaluation by Procurement and Safety and Health along with the manufacturer, JLG, determined the change to re-locate anchor point took place in 2006, but the operator’s manual had not been updated. </pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>The manlift was tagged out-of-use until resolution with JLG was determined. The manlift was subsequently accepted and the NCR closed Use-As-Is based on the following determination: The anchor point was purposely changed by JLG in 2006 to accommodate placement of protective padding on the top rail, and extensive evaluation of the applicable ANSI standard, OSHA Letters of Interpretation, and the JLG Operator’s manual by Safety and Health determined that fall protection is required when the gate is open or railing removed. WEMS can work within these guidelines with the anchor point as configured in the change.
Safety and Health entered an explanation in the operator’s manual stating that the anchorage point depicted on page 4-10 is not the orientation of the anchorage point on this lift, and the actual orientation is a 2006 JLG approved design change to accommodate a hand rail padding fixture/option.
The maintenance mechanics’ questioning attitude and follow-up evaluation subsequently resulted in JLG revising and re-issuing the operator’s manual. JLG can be contacted for assistance for any JLG Model #12SP manlift questions.</pre>

<pre>In accordance with ES&H Manual Chapter 6140 Appendix T2 Procurement of Material Handling Equipment, all such procurements, leases; related repair and maintenance services are to be coordinated through the Material Handling Safety Representative and inspected prior to use.</pre>
