Unique procurements require appropriate review and approval to mitigate risk of nonconformances. Also, additional vendor oversight is needed for items or equipment whenever procurement, fabrication, or receipt strategies are outside the normal process.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>Procurement Specification for commercial grade electrical panels did not require the Mixed Oxide (MOX) Facility's approval, nor provide for a means to resolve nonconformances. This contributed to the lack of wire separation in approximately 42 electrical panels that were provided by AREVA, Inc. and received at MOX.
It was not until Revision 2, in the 2015 document, that provided for a submittal schedule that required AREVA to disclose past nonconformance issues. As a result, there was lack of communication and problem resolution between AREVA and MOX regarding the wire separation issues that AREVA delivered prior to this date.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The condition could have been prevented if additional MOX oversight (i.e., Quality Assurance/Quality Control involvement and utilization of appropriate Field Engineers) had been employed in the complex procurement strategy. The lone MOX Commissioning Field Engineer assigned to the fabricators, although knowledgeable of the panel design, was not consistently thorough in his execution of fabricator oversight, due to his inexperience in performance of this task, as well as the volume of panels to be reviewed.
Because the panels were being drop shipped to AREVA, MOX’s Quality Assurance only reviewed Receipt Inspection Reports. Increased surveillance at the vendor’s shop would likely have been beneficial in ensuring proper fabrication.

<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
This lesson was distributed for informational training purposes to prevent these problems from occurring in future procurement strategies that are similarly complex and outside typical procurement processes.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab has a robust Subcontracting Officer’s Technical Representative (SOTR) Program which ensures a single point of contact for all subcontractors. The program also requires a Facility Design or Modification Review by Subject Matter Experts prior to the commencement of work by a subcontractor. </pre>
