                    <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>

It is important to communicate with vendors when items need to be identified by a valid NRTL Certification Mark. One should always be skeptical about unusually low priced items and particularly wary of electronic items that do not have an authentic NRTL label. Close inspection and knowledge of NRTL listing and labeling requirements is needed to prevent suspect/counterfeit item (S/CI) being introduced into the workplace.

                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>Alternating Current (AC) adapters identified as Model A1265 were requisitioned.  Samples of AC Adapters were received from the supplier prior to procurement.  these were inspected, and were determined to be a legitimate NRTL listed and labeled product. Based on that inspection, a material requisition was placed with the supplier for AC adapters. The AC adapters received were marked with Model Number A1265 and UL directory number E233466 which are consistent with a legitimate UL listed item. However, the items received were not marked with the correct company name and lacked the UL Certification Mark/symbol as required by the UL directory listing.

The S/CI problem was identified after release of the AC Adapters for use when an employee questioned the legitimacy of the items due to lack of an Underwriters Laboratories (UL) Certification Mark on the product.  Based on inquiries with UL, it was determined that the AC adapters received were counterfeit (reference UL Product Incident Report 2015FR-662) and a company-wide notification and return request was issued for the S/CI.

                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>Requisition shortcomings that contributed to the receipt of counterfeit AC Adapters included the lack of the model number in the line item and no requirement that the items must be NRTL listed and labeled. Closer inspection of the AC Adapters received identified other markings and labeling problems that are often indicative of S/CI (e.g., misspelling of the word ’equipment’ as ’equipmnet’ and all items received in the shipment had the same serial number on the label.

Both the Occupational Safety and Health Administration (OSHA) and the National Electric Code require the use of approved (e.g., tested and listed by a NRTL) equipment to help ensure product safety.  Potential safety hazards can be introduced if consumer electronic items are introduced into the workplace that have not been proven to conform to national safety and manufacturing standards.

                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>Below are the consequences of the above event.  They are provided for information and discussion purposes only. 

When purchasing products that are required to be NRTL listed and labeled:
- Ensure the model number is included in the requisition line item and explicitly state that the item being ordered must be NRTL listed. 

- Ensure the requisition is reviewed by personnel appropriately trained and qualified to determine the legitimacy of NRTL listings.

- Check the NTRL registries (e.g., UL Online Certifications Directory) and contact the NRTL or the manufacturer to confirm NRTL listings if there are suspicions of S/CI.

- Search the internet for S/CI concerns associated with the specific model number for consumer electronics.

- Maintain a questioning attitude prior to procurement and inspect items closely to verify a label’s authenticity prior to release for use.

        <h2 style="white-space:nowrap">JLab Preventive Measures</h2>
        <pre>Jefferson Lab has a robust Suspect/Counterfeit Items Program.  It is integrated into the requisitioning and purchasing systems.  High risk items undergo additional review and approvals.</pre>
