When performing telephone service/repairs, be aware of potential hazards concerning electrical shock. Only trained and qualified personnel are to perform telephone installation/repair services to avoid safety hazards that are encountered and ensure standard industry practices are followed.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On May 13, 2015 at approximately 0915 hours, three telephone service technicians were investigating a report of ¿no dial tone¿ on a telephone extension in an administrative trailer. The technicians removed the raceway cover at the bottom of the cubicle where the phone extension was located. Then one of the technicians moved the telephone wire that was connected to the phone jack and he observed a small spark. The technicians stopped work and contacted the Facility Shift Manager (FSM) to report the incident. The FSM contacted the Facility Manager Designee (FMD), the DOE Facility Representative (FR) and the Deputy Operations Manager (DOM) and at approximately 0945 the FSM, FR, DOM and FMD went to the trailer to evaluate the conditions reported by the technicians. Upon investigation, it was observed that the phone line had previously been crudely spliced (twisted together), secured with scotch tape, copper wire was left exposed, and the phone line was located adjacent to a 120 volt electrical receptacle box.
Standard industry practices identify the use of mechanically implemented wire locks to eliminate wire stripping, minimize exposed wiring and that technicians avoid placement of the phone line adjacent to an electrical receptacle box.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The spark was created when exposed copper wire came in contact with metal components of the cubical structure. The telephone cable run was not installed properly from the trailerҳ telephone patch panel to the telephone unit. Scotch tape was used instead of the industry standard wire lock devices and/or rated electrical tape to adequately insulate copper wire. Additionally, a potential for shock hazard (68VAC and 25mA to 35mA) is present in the telephone wiring/components during the telephone ring cycle which meets the hazardous energy requirements identified in National Fire Protection Association (NFPA) 70E.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Telephone service/repairs must be performed by trained and qualified personnel adhering to standard industry practices to avoid potential shock hazard. Telephone service personnel revised their Job Hazard Analysis to identify potential hazards and actions when performing telephone system service/repair. Personnel performing any future telephone work are required to take electrical worker training, and use appropriately rated PPE when handling energized telephone cable or remove/control electrical power prior to performing work.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab requires all telephone installations to be approved and authorized through the Facility Work Request System to ensure work is performed in accordance with industry standards.</pre>
