<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>

The Department of Energy¿s (DOE) Office of Environment, Health, Safety, and Security (EHSS) submitted an Operating Experience Level 3 Report summarizing electric shock events recently suffered by workers at various locations. This report is provided to increase awareness of the negative trend in an effort to promote good work habits.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>From January 1, 2012 through May 1, 2015, 249 ¿Electrical Safety¿ reports were filed in the DOE Occurrence Reporting and Processing System; 99 of these events (39%) involved shocks to personnel, with one fatality. (See the ¿Background¿ section of the attached report for further details.)</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>All electrical shocks are hazardous. Low voltage does not necessarily mean low hazard. Hazards can be reduced or eliminated when a thorough hazard analysis is performed, workers engage in safe work practices, and strict adherence to approved work procedures is enforced.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
All sites are required to file an Occurrence Report for events concerning personal injury, significant property or equipment damage, and environmental impacts.
Stop work orders must be initiated until the event can be investigated and re-evaluated for safety.
Equipment is removed from service until it can be repaired or replaced
