A portable spotlight, used during Security¿s nightly routine security rounds, was improperly stored after use which lead to damage to a government vehicle.
</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On July 19, 2015, at approximately 04:30 AM, following routine security rounds, a halogen spotlight (3,000,000 candlepower) was accidentally left on the front seat of a Government vehicle just outside the security guardhouse.
At approximately 06:15 AM, an officer observed smoke emitting from the vehicle. The officer responded with a fire extinguisher. Upon opening the door he observed the halogen spotlight partially melted (approximately 3-inches) into the seat (See Attached pictures). The fire extinguisher was not discharged as no visible fire was observed.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The halogen spotlight had been used for nightly security rounds for approximately five years. Security officers recognized the hazard (substantial heat generation) and habitually placed the spotlight on the floor mat, on its side, after use. This allowed the spotlight to cool down between uses.
In this instance the spotlight was set on the upholstered seat, face down which did not allow it to cool sufficiently. The security officer believed the spotlight was turned off, but was not sure.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
A fire watch was posted for the remainder of the shift to ensure spontaneous combustion did not occur.
Halogen spotlights were replaced with new LED spotlights which do not produce excessive heat.
