<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>A subcontract electrician was investigating an air handler component located above the ceiling tiles in a hallway. After accessing the workspace, he evaluated it for general conditions and discovered four exposed electrical conductors that extended out of a junction box. The ends were in the as-cut condition and electrical tape secured them to the conduit.
The electrician and the engineer with him stopped work immediately. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Conditions indicate that the configuration was created during building construction and missed during the inspection. LLNL requires that the load end of an electrical conductor be connected to the device receiving the electricity before the conductor is energized. Safe-ending with wire nuts and/or electrical tape is not an allowed permanent configuration, even if secured inside a junction box. </pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Immediate Actions Taken:
The ceiling tile was replaced and the electrician remained in the hallway to secure the workspace while the engineer made the appropriate notifications. The affected circuit was locked and tagged out of service, and a second electrician, wearing appropriate PPE, verified the absence of energy. The first electrician then safe-ended the de-energized wires with wire nuts, secured them inside the junction box, and installed the junction box cover to create an acceptable temporary safe configuration.
Scheduled Actions:
Remove the unused portions of the conductors from the conduit,
Complete an extent of condition for the building, and
Correct any additional unacceptable configurations that may be found.
Continuing Actions:
Include situational awareness into work task steps by observing the total work environment, all potential hazardsnot just those identified in the work package, and any hazards that may arise during work.
