Road conditions and surface variations can change throughout the work day. Operators of material handling equipment must maintain awareness and specifically address speed when road conditions change.
</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>In July 2015, water had been used to suppress dust on an access road leading to a sump well construction site. An operator of a Caterpillar 637 scraper was wearing his seatbelt and driving down the road when the right rear wheel slid down the slope due to wet/slick conditions. The scraper proceeded to roll over causing work to stop.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The scraper was being used to support surface construction of the new well site. Waste Management work in the area noted that ¿¿.When the tractor/scraper was up righted, it was determined that it lost approximately 39 gallons of fluids when it tipped over.¿ </pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
The Operator was transported to a local area hospital for evaluation/treatment. He was found to be uninjured (except for soreness), and released for full duty.
The NNSA/Nevada Field Office reported the fluid release to the State of Nevada, Department of Environmental Protection. Remediation of the spill was then performed
A work time out was called for operating heavy equipment in conjunction with Environment release.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>In accordance with ES&H Manual Chapter 6140 Appendix T1 Training Requirements for Material Handling Equipment ¿ ¿Jefferson Lab requires qualified material handling operators to be approved by their Supervisor/Subcontracting Officer¿s Technical Representative (SOTR)/Sponsor.¿
Pre-Job Briefings are also performed to determine equipment operation, associated tasks being performed in an area, and safety for specific locations.
