An employee, using only one hand to unplug a piece of equipment from a power strip, received a mild electric shock.
</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>An employee received a mild electrical shock from a standard three-prong 110-volt power plug when he attempted to unplug a piece of equipment from an unsecured power strip with one hand. On the first try, he tried bracing the strip with his thumb, while pulling with his palm and fingers. His hand then slipped off the plug, leaving the prongs partially exposed in the power strip. On the second attempt, his finger contacted one of the now-exposed prongs resulting in a mild shock.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Based on past success with this one-handed technique, the employee had become complacent and did not look for a safer way to accomplish the task. The employee had both hands available but was focused on the task not the hazard.
The investigation identified two errors that contributed to the shock. First, the employee used only one hand to unplug the equipment. The power strip was not secured, the employee placed his thumb on the power strip for leverage while using the hand to grasp and pull up on the plug. This necessitated having the thumb and index finger exposed, in close proximity to the energized prongs.
Second, the employee chose to approach the plug from an angle where only one hand could be used, rather than repositioning himself so both hands could be used (see attached photo).

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Hazards related to 120-volt electrical systems in homes and offices are commonly understood therefore, special instructions, training, signage and supervision are typically not needed when plugging in or unplugging equipment. Employees were reminded of the proper techniques, which include:
1. Ensure cords are not frayed/damaged and all prongs (including ground) are in place.
2. Ensure hands and work area are dry.
3. Ensure prongs are not exposed prior to grasping plug.
4. Maintain line of sight with the plug/outlet.
5. Grasp the plug, rather than the cord.
6. Ensure socket/power strip is secure, or use both hands.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>ES&H Manual Chapters 6105 Appendix T1 Office Safety Requirements, and 6200 Appendix T3 General Electrical Safety Guidelines provide an overview of general electrical safe work practices to be used by everyone at Jefferson Lab. It is recognized that increased awareness of these practices helps maintain an electrically safe work environment.
In accordance with ES&H Manual Chapter 2210 Overarching Responsibilities to Ensure Compliance with ES&H Requirements, all supervisors are responsible for ensuring that ES&H program requirements and practices are understood and implemented by individuals under their authority.</pre>
