<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Prior to commencement of work, ensure controls are in place to mitigate "line-of-fire" hazards that may present if unexpected or upset conditions occur. In this instance a SWAG Port-a-band table saw blade was unguarded during the evolution cycle and extending tool (push rod) was not used during the cutting process.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>A worker, wearing leather gloves, was removing excess metal material from a weld test coupon (a 3/8-inch thick carbon steel plate) by manually pushing the material toward a band saw blade. The worker was applying forward pressure with the hands in line with the blade. After the cut the worker inadvertently pushed his left hand into the rotating saw blade which resulted in a laceration to the sided of the hand and required seven (7) sutures.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Contributing causes of this injury were that the worker's hand was able to come into direct contact with the unguarded blade; and use of an extending tool (push rod) was not employed.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
All departments discontinued use of SWAG Port-a-band table saws and stands until an evaluation of the tool, guarding, and the availability of other options had been completed.</pre>

Jefferson Lab recognizes that Machine tools are inherently risky. An Operational Safety Procedure (OSP) is written for each machine tool and read by machine operators prior to use. "ES&H Manual Chapter 6121 Appendix T1 Safe Operation of Machine Tools" provides the safety rules applicable to operating machine tools at Jefferson Lab. It is based on Occupational Safety and Health Administration (OSHA) regulations and American National Standards Institute (ANSI) standards. The machines' Operator Manual also provides valuable information and are used in preparation of the OSPs and training</pre>
