<pre>Periodically re-evaluate waste program job duties, procedures, checklists, and other infrequently performed or complex functions for continual improvement, conduct of operations, and training needs.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>During an Extent of Conditions review, to assess all waste containers shipped from the Idaho National Lab (INL) to the Nevada National Security Site (NNSS) between September 2014 thru September 2015 it was found that three separate shipments (September 23, 2014, January 12, 2015, and April 7, 2015) each contained one non-compliant shipping container. The shipping containers contained fissile waste which had not been identified on the approved waste profile nor packaged in a manner that was compliant with the NNSS Waste Acceptance Criteria.
INL disclosed its findings to DOE and on October 7, 2015 Nevada National Security Site, suspended shipping privileges.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>1) Training did not prepare personnel to fully understand the NNSS nor the INL waste profile requirements.
2) Waste Management did not ensure their expectations, to elevate concerns, were understood and enforced.
3) Waste Management procedures did not ensure personnel verified that shipping containers and shipping methods were compliant.
4) Waste personnel did not comply with Waste Management procedures.
5) Required reviews were not clearly delineated in procedures.
6) There was no method for feedback to be provided when problems were encountered.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Programs should periodically re-evaluate job duties, procedures, checklists, and infrequently performed or complex functions for continual improvement, conduct of operations, and training needs.
Processes should include adequate criteria for the review of shipment activities that strengthen the process to identify errors before waste is shipped.
Management should reinforce the need to elevate concerns and questions without fear of retaliation.
Management should clarify the expectation to seek technical or management support when complex situations are encountered.</pre>

At Jefferson Lab, shipment of chemical or radiological waste is restricted to the ESH&Q Division. Shipment of any hazardous material offsite is only authorized by Jefferson Lab's Industrial Hygiene Department, Radiation Control Department, or Shipping and Receiving Department per ES&H Manual Chapter 6600.
Any chemical or radiological waste collected for disposal must be coordinated through ESH&Q (specifically Industrial Hygiene and Radiation Control Departments) to ensure that proper storage/shipping containers are used.
