<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>It is imperative to implement corrective actions from previous events in a timely manner. Once implemented SOTRs should share relative information with their contractors.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On December 8, 2015 at approximately 3:00 pm Facilities Management was notified that the subcontractor installing a new chilled water piping system hit an electrical line conduit that feeds the south side CEBAF Center parking lot lights. At approximately 4:00pm, notification phone calls were made to the ESH&Q Reporting Officer and the Facilities DOE representative. The Director of Facilities was able to have a site wide e-mail sent informing the JLab staff of the event and that the lights would not be operational.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Root Cause: Failure to follow the approved dig permit for the identified work- Contractor was using a back hoe within 24 inches of the identified utility. The markings were clear and within 1 foot of the located utility. The contractor had used the hand dig method of potholing to locate the utility and had placed a PVC pipe over the utility to mark the ground as a reference.
Contributing Causes:
1. Failed properly lock out the photocell for the parking lights as indicated in the ES&H Manual before starting their dig work. They were using the photocell timer as their LOTO method.
2. Failed to get approval from their SOTR to use the shelfing method/back hoe within 24 inches of the identified utility.
3. Previous Lessons Learned and corrective actions were not shared with contractors and enforced in a timely manner

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>1. The Data Center UIM project subcontractor was instructed by the JLab project team that the known underground lighting utility lines would be de-energized and Locked and Tagged (LO/TO) by JLab prior to doing any further excavating where the excavation crossed the utility. As a precaution, the FM&L project team used this same corrective action to safeguard a second project which was in the same area prior to doing their work.
2. Issue a safety Deficiency Response letter to the contractor.
3. Update the language in the dig permit to ensure that the requirements for mechanized equipment is clearly stated.
4. Share the event and lessons learned with FM&L at next SOTR meeting to ensure that everyone is aware of this event and to ensure the SOTRs are sharing relative Lessons Learned with their contractors. </pre>
