<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Evaluate pedestrian walkways particularly after an extended period of inactivity (e.g., holiday, long weekends, and project shutdowns). Look for hazards that may be a result of changing climate conditions. </pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On January 04, 2016, after a two week holiday which included several days of variable temperature and precipitation, at approximately 5:55 a.m., when temperatures were approximately 31F, an employee was walking on a sidewalk adjacent to the north side of an office trailer. Rather than make a right turn and proceed up the ramp to the trailer entrance, the employee continued along the roadway. According to a witness, when the employee became aware he was standing on ice he paused trying to determine the best route to reach a dry gravel area. As he stepped forward, his left leg slipped out from under him. His right knee appeared to hyper-extend and he went down, landing directly on the kneecap. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The north side of the building receives limited sunlight during the winter months. The down spout off the north side of the building causes water to flow from the roof onto the gravel surface. Low temperatures cause this water to freeze. Snow and ice also tend to buildup in the area which results in a significant hazard to pedestrians.
It was the employee¿s daily habit, good weather or bad, to leave the sidewalk and pursue a travel path across the graveled surface to enter the back door of the building.
A maintenance crew was not scheduled to treat the area until later in the day and no physical barrier was in place to prevent pedestrian travel.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
The injured employee was attended to in-place and transported to the local hospital where he underwent surgery to repair his fractured kneecap.
A site-wide safety briefing was held following the incident. Actions and reactions were discussed. Based on the information gathered the incident was determined to be reportable and appropriate notifications, reporting, and investigations were made.
Cones were placed around the incident area to prevent additional foot traffic. The cones will remain in place during inclement weather (i.e. snowy, icy).
Ice melt salt was applied to the area.
During inclement (i.e. snowy, icy) weather vehicles are now required to park in the main parking lot and pedestrians are only to utilize the middle corridor between the northern and southern trailers to access all area buildings.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab's Subject Matter Experts have provided the following statement:
"In the event of inclement weather, members of the Emergency Management Team make a determination of Lab status (such as delayed opening or early closure) based on local forecast conditions. Designated members of the Facilities Management & Logistics Division, supplemented as necessary by subcontractor personnel, implement an established plan to clear and/or treat roadways and walkways. Employees must remain mindful of the hazards associated with icy conditions, and utilize cleared/treated walkways to reduce the risk of slip/fall incidents."</pre>
