<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>The unmarked pinch point at the end of a freight elevator¿s bump railing gave the incorrect impression that it was a safe location to place a hand. While doing so an employee sustained a laceration when the inner cage door was lowered.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>An employee was resting his hand over the bump railing of a freight elevator when the inner cage door of the elevator was lowered. A bracket, attached to the inside of the elevator inner cage-door, struck and the employee¿s hand causing a laceration. A co-worker immediately transported the injured employee to Health Services for evaluation and treatment.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Unmarked pinch points are generally areas that are not apparent, or have gone unnoticed. In this example, the elevator cage door was most often viewed in its fully-open or fully-closed positions. This left the impression that pinch points only existed between the door frame and the end of the railing. The pinch points between the protruding bracket and the end of the bump railing, remained un-noticed and thus unmarked. The lack of markings created the impression that a hazard did not exist.
Hardware geometry or location can cause a person to perceive that a gap is ¿big enough.¿ A cursory look at the elevator¿s bracket location relative to the railing gave the impression that the bracket was past the end of the railing thus there was no pinch point hazard</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Unmarked pinch points on this and other freight elevators are to be reviewed, identified, and appropriately marked. Additional actions, including physical modifications, are being taken as appropriate.
Employees are to notify the Facility Point of Contact (the FPOC¿s name and phone are posted in that building) if they perceive an unmarked pinch point hazard, stop work, and take appropriate action to prevent the pinch point from causing injury or damage as appropriate.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>If you recognize an unidentified pinch point, initiate a Facilities Management Work request. Identify the location and the concern, request that appropriate notification or restrictions be applied.</pre>
